Besides a truck mechanical failure, etc., What would be the reasons why a driver doesn't deliver an assigned load to it's destination?
A) Running out of hours = Wouldn't driver know ahead of time if they don't have the hours to make the delivery?
B) Mechanical failures/traffic accidents? = Well, that is self explanatory, but if a truck has a failure, how could they get to a terminal/drop yard so another driver could pick it up? The trailer can't be transferred out on a highway!
C) After picking up load, Driver wants to celebrate Hannakuh, so says "I'm getting all feklempft, so you take it!"
D) All of the above
E) None of the above

The three reasons you stated are some. Sometimes a load might not deliver for a few days and a driver can get it to a yard and then have another driver deliver it, rather than sitting on a load for a day or more.
Or you requested home time and that load will get you close, but you're too far off the route to take it home with you.

Electronics and pharmaceuticals can run up near 7 figures. $80,000 is chump change
Many of the meat loads I haul are around $150,000-200,000.
When I worked for Digby, we hauled alot of suits. You can fit about 10,000 suits in a load. Do the math there. -
Most expensive thing I ever hauled was a trailer load of bras. The are light, small, you fit a whole lot in there and they are expensive. -
I had a $1.2M electronics load once. Came from Samsung in California and went to a Walmart DC. The company made sure to tell me that under no circumstances was I or my student to leave the truck unattended. We even showered separately, but other than that the truck never stayed parked for more than 15 minutes.

A good example is a delivery that goes to NYC. Since a highway truck simply won't fit in a lot of places in the city, they will have you drop it at a drop yard, and a local with a smaller tractor and local knowledge will finish the delivery.
