Where can you find the DOT forms online?

    I need the physical card, and long form paperwork. I am looking through the texas DMV site, and fmcsa.dot.gov to see if I can find it. They are not exactly user friendly so wanted to check and see if anyone here happened to know where I could get them.

    Any clinic that does DOT physicals will have the forms and the card, and will furnish them.

    just so that you know, a paper DOT card is not acceptable...it has to be the cardboard type given by the clinic/doctor/trunking company....now i should back this up a bit and say, "here in my area, and the states i deal with, a paper DOT health card is unacceptable."...maybe texas allows this, but to be on the safe side, go to the clinic....or even look for the local ATA affiliate trucking chapter in your state, as they will have just about ANYTHING a driver needs....

    Actually the CVS I am heading to does not have the form otherwise I wouldn't have asked

    A photocopy, on paper is all I've ever used,

    no problem in the lower 48 !!

    Print the Certificate on cardstock paper. It is permissible to reduce the size of the Certificate.

    in other words, even by the DOT, ordinary paper is not allowed.....

    You can print on photo paper or cardstock like rerun says.

    It shouldn't be an issue like frontrange says, the doc supplies them.


    You get good at it, then you can start printing money, lol.
