History Records Request from US Xpress Fax or mail?

    I was OTR for 2 years and I thought I have a good record but when I went to US Xpress the 2nd day they said I was not hired (can't give me the reason) well I learned I need to fill out a form and send it to them, I got the form (history records request) and filled it out but there is no fax # on it do I have to mail it? there is address but it doesn't say send it to this address!! also I'm sure I have nothing bad in my history records that's why I can't wait to find out why they didn't hire me. could it be cuz I'm not too white? when I tried to get to the bottom of it one of US X safety said I think weren't hired cuz u don't have hazmat!! but it's not required I said? I don't know u have to send the form he said. I was denied hazmat over a year a go cuz I was not citizen but now I am so would that be the problem cuz I was denied hazmat? IT'S NOT REQUIRED!!
    if anyone have the fax # or have some info might help plz do.
