DMV and your DOT Medical Long Form

    Might want to check with your State's DMV about the new medical requirement to your CDL

    New law takes effect Janruary 31, 2012.

    I know Washington State will require a decleration of "Inter" or "Intra" state commerce will be required and be noted on all new CDLs (Intra will be noted as a "U")

    Also in WA drivers will be required to bring their current DOT Physical Long Form to be scanned and it will become part of your driving record.

    WA info states that this will be done on all CDL drivers next business visit to the DMV after 01/31.. It says nothing about having to come in specifically for this, or doing it before that date.

    You may want to check with your state and see what the requirement is in your particular state. I do remember reading something that some states may have a temporary federal waiver to comply as they are behind in implementing the new law.

    I have not been able to find out if this is something that will have to be done each time you take the physical, or if your physical experation will be tied to your CDL experation.

    I know some people only get 3 or 6 month Medical cards. Im on an annual renewal for my sleep apnea. I figure the vast majority of drivers are on the 2 year med cards.

    BTW in WA if you decalre "Intra" the medical form on file will not be required. However, if you are caught at a scale with any "Inter" state commerce loads you will be parked and put out of service.

    I wonder how this will work out once implemented?

    Most (not all) at the DMV are pretty slow, so forging a long form shouldn't be hard to do.

    I got that letter from DMV also, and was thinking the same as U2 if the medical long form updates itself when you bring a new medical form every year or two to DMV. if you are on the road and it is time for your physical i figure prime will send you to the doc and once you pass they send your medical form thru the fax to DMV so it can be updated. I wonder will it show on the license that you pass or they just keep it in the records where you don't have to go to dmv every year or two to change status on your license? Do you think that when DMV downgrade your license if you do not send the medical form after the 31st downgrade it to a cdl permit? I would not have a truck driving job so i would not be able to claim i am a interstate or intrastate driver.

    I keep hearing about a long form, Is there another form other than the Long form?
    Reason I ask I have a dot physical comming up in Feb. and want to make sure I get the proper one.

    There is a long form and the actual medical card most keep in their wallet. The long form and card is integral - you won't take a DOT physical and get one without the other.

    The long form summarizes and notes the entire DOT physical - height, weight, blood pressure; vision, etc...

    A medical card is essentially a "proof" you passed the DOT physical.

    I live in Michigan and took the CDL written tests two days ago. I had to answer the "interstate or intrastate?" twice on the application. My guess is U2 is totally right--all states are going to require some kind of acknowledgement from CDL applicants or holders in one way or another.

    Here is what VA. is sending to current CDL holders. My CDL expires in Aug. 2016. So I am going to have to renew 2 years and 7 months earlier then I should so they can scan my med. form into the computer. I called and asked if I can just have them scan my form in and was told NO.:biggrin_25510: I must have a renewal done by 01/30/14. Sucks cause I paid for the 8 year fee :biggrin_25510: More ways for the state to make money.

    VA. also decided this year to tack a 5.00 fee onto anyone that does not renew by mail, guess what CDL holders CAN'T renew by mail. More money out the window.

    I just swapped my cdl from tx to mo 3 days ago. I asked them about this and they had no idea what I was talking about.

    wow fmcsa really wants us drivers to do all that. me personally thinkis it's unfair.
