If everything is right, plastic washer on the antenna in the right place, meter and radio working properly, then it is the tool box. I can't get a good SWR mounting the antenna anywhere on my PK bed, much less on the tool box setting in the bed! I had to go to the front fender, then eventually I just drilled a hole and mounted it in the center of the roof. If you can find somebody with an old 102 steel whip, swap it for your antenna and see what happens. They work a little better in a bad rf ground situation like I think you have. Four bolts into the bed does not make a very good rf ground, which is what I think your problem is. (I am not a tech, but I've been messing with this stuff for about 40 years and have seen this same problem before.)

If you ran a very long ground wire, over 2ft, it could be effecting your output.
