I've got a bunch of questions lol. I've read a lot of the threads, and forgive me if i ask something that's been answered 1000 times, but i want to make sure that i have everything together when i head out to Springfield.

Do you have to fill out a new application when you get there? and what happens if they cant contact a former employer? a shop i worked for, under the table i might add, went out of business in 09, i have a contact # for the owner, but nothing else. do you need 10 yrs employment history even if you've never held a cdl?

Do they consider traffic citations criminal offenses that need to be reported under the ever been convicted of a crime? i have 3 actual charges from years ago that i listed on my app and was upfront with the recruiter when we talked about it, however in my crazier days i chalked up quite a few tickets, mostly for stupid ####, call it small town cops and fast cars lol. never a dui or anything wreckless. i just want to make sure that everything that needs to be brought up, is. I assume they run a pre dac/mvr to make sure we meet the guidelines before we are accepted?

i asked some medical questions in the sticky'd thread, and i got the answers, sounds like ill be doing a sleep test lol, my bmi last time i tested is 26, but my neck is right around 20".

ive got a ton more, but thats a start. like i said, just want to make sure i cover everything and have everything in order, ive been trying to get this together for the last 4 or 5 years, the timings finally right, i just dont want to give up a decent job to get out there and find out i quit for nothing.

Anyone else heading up during that week?

    YES

Make sure they know it is out of business, and bring your tax returns that show you worked there.

I do believe they do. Make sure u print out the online app and take it with u to oreintation also, helps with making sure everything is consistant.

Make sure u tell them EVERYTHING, all tickets and any run ins with the law. No they do not run a Pre MVR, but will when u are there, and if it shows ANYTHING that u have not told them you could get sent home very easily.

no,traffic citations are not considered criminal offenses. getting a speeding ticket in ANY vehicle for going 15 or more over the limit can be viewed by a company as reckless driving, even if thats not what you were charged with, and could disqualify you.

yes you need 10 years of employment history. this applies MORE to non driving jobs...you only need a few years of driving history IIRC. if they cant contact a previous employer, it may or may not be a deal breaker, depends on the company. having phone numbers of people who can verify your activities during that time (close friends) would be an excellent idea. id say try to contact the owner yourself, and tell him your predicament, that you need his help...maybe HIM calling THEM will help??

You will have to fill out a new application, even if you already filled one out online.

Nothing you can do about a former employer going out of business, they most likely won't even try to contact any employer other than your current/most recent.

10 years is driving history. You need at least 3 years for non driving history. From Prime's site:

"DOT regulations require us to obtain the names and addresses of all your employers/contractors for the past 3 years. For the 7 years preceding that 3 years, we need the name and address of the employers/contractors for which you were the operator of a commercial motor vehicle, including employers/contractors where you only attended orientation. For each employer/contractor provided under both these requirements, we must have the dates of employment/contract and the reason for leaving."

They need criminal history, ie arrests, felony convictions. Not sure about misdemeanors. For driving history, you only need the past 3 years. Again from Prime's site:

"History of traffic accidents, violations and/or convictions from the last 3 years (including DUI or reckless driving conviction and license suspension)"

    Good luck!

Yep! You have some good questions! It looks like they have been answered here. Best thing to do is be upfront and honest about everything. Once they see that you are truthful with it all, it will be a lot easier for them to go through the paperwork and get you signed on.
    Good luck!
