PA Medical Card Self Certification

if it's like MI,you need to take your new medical card to the DMV so they can update the system.

 

I just downloaded the form today here in Mass... Here anyway... you can mail it in... take it to MV... or fax it in...

 

Every driver with a CDL has to self certify before CDL renewal . I got the form from the DMV . Some states may have it available online . http://www.fmcsa.dot.gov/registration-licensing/cdl/faqs.aspx

joseph1135, I live in Pa. and I just faxed mine in last week to the phone number that they gave, and said you could mail it in too. hope thats helps you out,

 

Here in Michigan, I got a letter from our DMV. Went to the website and, using my smartphone, took photos marked the appropriate boxes and, volia, it was done.

 

I did mine when I renewed last Sept. as I still hold my CDL in PA. The PADOT office had a form to fill out so I sent that in with a copy of my Medical Card......got a notification back that it had been received and all was good.
