How much difference is there when its up to temp? How cold is it in the video?

Yeah plug it the block heater. Mine started better than that this morning and it was -10

Is the ecm program original or has it been tuned? Is it a ddec 4?

Rebel this is the same truck I PM you about several months ago. The people have never come to pay bill. This truck has been out of framed and new bull gear, reman head, new injectors, etc.. Ran overhead 3 times. Pulled front cover off to check timing. New timing sensors. I do not know the history on truck or ECM.
After it gets running for a few minutes everything is clear. -
White smoke can be from low compression, air in the fuel, poor injector spray problems, or low fuel preasure. A Detroit has a cold mode program and should start very easy compared to most every other engine. We found some engines, N14's, started better when on cold mode that when not.

Check your oil temperature sensor...if its going bad it'll throw out white smoke like you're describing. Also the intake air temperature sensor can do the same because it also affects cold starts just like the oil temp. sensor. What I usually do after an overhaul is change all the sensors so you know that you're on a clean slate. Those two sensors should solve your issues. Hope this helps.
