The 75 hours is minnimum with the instructor. Having a CDL with 6 months experience 5 years ago, if you pick it back up where you left off I don't imagine it being much more than that. That 10k miles is credit for doing that part of the CDL course (PSD).
The TNT course is 40k miles. PSD grads get that 10k credit so they do 30k miles with a trainer. More than likely you will not run 10k during PSD, but you've already had a couple weeks driving with Prime and that puts you a little higher on the ladder than someone coming in from another trucking school who has no experience. Those guys do 40k.
The 30k-40k is a minimum also. Hitting something, or having a problem can have extra training miles tacked onto your plate.

If you have your medical card that is self certified in my state, does that mean I don't need to take a physical at prime orientation?

Ok. That's two completely different answers.

Sounds to me like they are saying that no, Prime won't test A1C during the physical. They will test your urine for glucose. If you are currently diabetic, under treatment and not insulin dependent, be prepared to show them a current good A1C result. If you can't show them, they will make you go to your personal doctor and get one because they aren't going to.
Bottom line as I see it, no needle stick for A1C, but dip the strip for pee-pee sugar. If you aren't diabetic, no worries. Don't sugar load before the physical and you should be fine.

If you're a type II diabetic, you'll have glucose in your urine as your body tries to rid itself of excess sugar. It tells a doctor what's going on now... A1C gives them a more historical look at what's been going on in past months. A1C also takes some time, and requires lab work... so if you have that information it's going to tell the doctor what's going on. So both answers are correct- it's been some time since I had a physical AT Prime, and that time they didn't ask for an A1C result, but that has been the case when I've visited Concentra for my DOT physicals the last few years.
