Improper chaining allowed the dolly to move under the load in the previous turn, then stayed in that "drifting" position.

It was hydraulically steered. Tell us how improper chaining affects not centering the steering after a turn.

The dolly while it looks like it has hyd. steering capabilities, doesnt seem to be using it. Might be part of a six axle articulated system that is just being used as a single dolly for this job. The dolly cheated quite a bit around the left hand turn, (a mechanical system would not have allowed this). My guess is that the dolly slipped a little during this left turn. a 3 axle group like this turns very hard when its heavy. If this is the case, cross chaining the front of the dolly to the bottom of the beam would have kept the dolly straight. The driver did see what was happening at the end but not soon enough to stop. Just my observation.
