It all depends on which plan you choose.
There is no 'one size fits all' when signing up for health insurance.
There are so many options to choose from, and that won't be any different with any company. -

deductable paid first. BUT they give you HRA dollars that get spent first and apply to yearly deductable. These dollars also roll over to next plan year if unused..
Amount depends upon plan chosen and if the employee and/or family participates in a wellness incentive program that adds more in this account. FSA dollars can also apply towards deductable , but those need submitted as claims by you to insurance.
My totsl HRA dollars do not cover ll the yearly deductable , have yet to even meet it or need to really. certain visits are paid outright by insurance too but do not apply toward deductable(yearly physical and certain blood work)higgybaby Thanks this.
