Hmmmm Tough call there. Depends on the road. 4 lane or 2 lane.
The deadliest is in TN. 2 lane road (can't remember the hwy number) the loggers used to run before I-24 was completed. Right down the street from Mont Eagle.
Highest fatality rate of any grade in America. -
Where ever you get 1/2 way down one and find your brakes fading. Most of the hills I am familer with are in the west. Donner, Cabbage, Vail, Ashland, Forth of July, a few out in the sticks in AZ and NM. Lots of others. As far as the most scary I wouldnt consider any of them scary unless I was going down without any brakes.

I consider all of them scary...with brakes. In the winter
At least the ones that tend to ice up.

The 395 Southbound N/O Mono Lake. It's an 8% grade for about 10 miles.

a little two lane in PA... not long but 11%... trainer wanted to see if I would panic. I didn't, just started over in 5th with the jake on and tapped the brakes a few times, no biggy.
and a long time ago... rattlesnake pass I-84 between tremonton utah and snowville utah... before the interstate was finished it used to claim its share in the winter. steep, curves, and ice... a real pain in the behind.
used to pick up extra cash unloading semi-trailers so the wrecker could stand them back up after the crash and burn... -
Man it wasn't scary but it was a fresh experience and I held my breath most of the way I think.
It is US 33, in WV from a little east of Elkins to Seneca Rocks. 10% grades where you are at the top and can see all the way to the bottom where there is a 15mph hairpin turn and you got 80k on, cliffs on both sides.

Cabbage, Donner (not so bad), the Grapevine, Monteagle (if your not paying attention), most of the 2 lane roads in any Mountain range has it's fun spot's. Quebec has it's fair share of interesting area's too

white bird in idaho. it's been a while since i've been up there, but as i recall it's 8 miles of 6 to 7%, it's a long and steep son of a gun.
