I injured my back in Iraq, then again in 2014, and it has never been the same, would I be eligible for a DOT medical card, or would I get DQ'd due to the pain?

Don't mention the pain. If you on certain narcotics for pain that are banned then that disqualifies you.
I injured my back in the military, so many times slept halfway sitting up in a folding beach chair I carried in the sleeper. No one knew about it except me.

It's been a while since I went to Prime's school (for a week), do they do a flex test like the military does? I am not on any pain meds for it, it just flares up now and again. I am, however, on inhalers for asthma, and will be on a CPAP for apnea.

I can't answer that about Prime; just know they're more strict than most companies.

Well, I meant the Med Examiner in general, not specifically Prime

You are sure starting off on the wrong foot, 1st, starting a profession that almost always results in back trouble ( going from sedentary, to unloading a load of freight) and 2nd, lying about it. If you have documented back trouble, and don't say anything, an insurance company may drop you, then what? IDK, sorry, doesn't sound good for you.

    Big Don Thanks this.

Do you plan to attend a trucking company school or a private cdl school?
Many refrigerated companies these days are "driver no-touch freight." This is good news for anyone with back problems.
Since you're on CPAP machine; companies want their drivers to use one that stores data and that data can be downloaded. They will want to see the last 90 days of compliance any time they check it.

Google for that information, because I don't have any names.
Maybe work something out with that small cdl school in Los Angeles; "Khan D' Kulia Trucking School."
