I'll guess it says "inTRAstate only". Don't need a medical card intrastate in many states.

This reminds me of our local Dr that does most of the DOT physicals around here.
Last two times he did mine, he made me squat down and walk like a chicken. I thought it was odd but I'm fairly limber so I complied.

I was talking to a older gentleman about DOT physicals and he said "did Dr White make you walk like a chicken"? I said yes and he said "I told him that if I got down that low, I'd never get back up and in all my 30+ years of driving a truck, I've never had to walk like a chicken to fulfill my duties". He got his card so I guess Dr White let him slide.

It's the driver's responsibility to maintain his credentials. He should not be driving at all until he gets one. Legally he does not have a valid CDL until he does.

So here in PA, we get the signed form from the doctor and fax it to the state. Does the state actually verify the form with the doctor? How do they know the form is legit?

Any time your medical card expires, your license is downgraded and it is illegal to drive a truck. You can get your medical exam done in any state. You will need to fax the paperwork to your state.

Google - "your state" and self certification.

    medical card expired?

Go down to the Revenuer (DMV to you yankees. Yes we use that word here.

Sign a oath swearing your CDL is now Tier two. You are not required now to hold a Medical Card at the moment once you sign that.

When you get a Medical DOT card with long form etc back valid go back and reswear your CDL back to Tier One for Interstate and Intrastate commerce.

Do this asap. You do not want to run a risk of the Government suspending your CDLA for not having a valid DOT medical tied to it.
