I had to replace some logistics posts in my dry van trailer one time and I just work on it where I park it. I sheared the tip of a T25 torx bit off and just tossed it out the back end of the trailer onto the gravels out there thinking nothing of it. About a month later I had a trailer tire that was low. I found the leak and wheeled into a Love's to get it patched. And what did they pull out if it???? The broken half of that torx 25 bit I tossed on the ground in my parking area a few weeks earlier. LoL... I knew it was the same one because of the way it sheared off at a weird angle.

I was fueling up in Jane Lew, WV. years ago and looking at a drive tire. Something was stuck in the tire, at closer look it was one of those cross shaped valve stem combo tools. I cursed a bit and got out my spray bottle of liquid soap while I worked the thing around. Lucky me, it didn't penetrate.
I felt real good about noticing that since a lot of times that piece would have been at the bottom of the tire not to be seen and deliver the opposite luck some time later.
Also had the same wrench as the OP but larger (9/16) stuck in the tire same way. All you could see from outside the tire was two small metal dots, turns out it was the open end part of the wrench. -
Caught this razor blade before it went too deep.
I’m still kind of amazed how it went straight in like that. -
A table knife got me once in a drive tire.

Deer antlers in ag equipment, don't know about truck tires.
Had a cut fairly heavy piece of chain link through a drive in a rescue truck the shop foreman brought me, the tire was off the bead, but I didn't figure it out until my last stop. The puncture was repairable. -
Before my motor let go, I had quite a bit of blow-by. Shop time was scheduled for the overhaul, but still a few weeks out trying to ride it out until I had some planned time off. Anyway, I'm pulling away from a stop sign making a left hand turn when I hear a tire get punctured and a bad leak. So, I make my way a few miles up the road to a tire shop, and while they were working on the drive tire they noticed a big oily mess in their lot, and all down the side of the engine. Blow-by had blown out the fill plug, which I ran over with my drive tire. They gave me a ride up the road to a Mack parts store to buy a new plug, and by the time we got back the tire was replaced. I proceeded to safety-wire the fill plug with some bailing wire after that. It popped out one more time before I got the new motor, but it didn't go far...just had to pull over and put it back in. Looking back, it was probably a good thing that it punctured the tire...no telling how much worse it could've been had I not realized I was missing that fill plug...oil splashing out, hot turbo, could've been bad.Slowpoke KW and rollin coal Thank this.

One of our trucks rolled in with a near flat inner dual, found the problem to be a flat screwdriver that had gone through the treadwall of the outer dual, back out its sidewall and into the sidewall of the inner dual...screwdriver was still in it and the outer tire still was holding air! Really wish I’d taken a picture of that one
