    I got one of those this week too. I did not fill it out and had explain to the uninformed broker that it only applies to 2000 and newer trucks.

    I agree in theory, but id add in an exception. If its a brand new authority, say under a year old, you can't count on the reliability. Far too many guys run out and get a worn out truck immediately get their authority and have no cash on hand to get the truck in decent shape.
