One of my co-workers father retired. Ran a grain truck for a neighbor, something broke causing him to go underneath to check out something.
They found him wrapped up around the driveshaft. Clothing got caught when the truck moved.

Better safety equipment cut down the fatalities is all. Little things like seat belts and air bags.

Sure forgot to set brakes. I see it looks like flat ground too maybe. I don't know. If it will roll it's because the brakes are not set.

I remember crawling under my bulk tanker after chocking it all after the parking brakes failed to hold it on dry steep incline at Rockville Fuel and Feed in Rockville Maryland. Adjusting the brakes harder manually until it stays put long enough to unload the #### thing.

That's scary. I recall at least two fatalities from my previous employer of people who failed to set the brakes. Both men were completely inexperienced, in a big hurry and completely wreckless. Both were recent 'new drivers', both with less than 1 yr. experience who were turned loose to tow, load and unload oversized loads, primarily road equipment all over the lower 48 and southern Canada with the rare load going into northern Canada. Alaskan loads typically went by ship. Oh man, our death-injury-damage-accident rates were bizarre. Many 'spot inspections' in many states and Canadian provinces due to our horrendous record.

Forgetting to set the brakes I would think is more common than people think. I'd have to say it may come down to bad training and/or trucking not being for you. Not saying that was the case in this particular accident but for most. The Two!
