I would take my tools with me and pull the turbo exhaust off and check the injector if it's clogged with soot. The delta pressure sensor as well. Imap sensor. Open the tube the egr feeds to the intake check for any signs of moisture, coolant will cause that.
There is a series of videos on YouTube by rawze called egr tuneup cummins isx it is a 6 part series and is very worth watching. He demonstrates on a cm871 but the same principle can be applied to the later engines.

I appreciate the feedback. I am trying to get a good day cab tandam axle truck that my drivers will use from Portland, OR to Seattle, WA that will run about 80K miles per year. I am looking for something for about $50K. Any ideas?

You're probably in the right ballpark honestly. It is geared right and is probably decent to drive. There is no magic bullet anymore, everything is a gamble. Some folks say cummins has issues....well, yes, which of them don't nowadays? I prefer to know what the issues are likely to be than wait 3 weeks to get in to a freightliner/Detroit shop.

I have never known a dealer that would let you take a box of tools and disassemble their truck.

That is hardly disassembly. If I am going to dole out 50k for something you can let me take a few bolts off here and there to inspect, I will put it back together. I am not asking to take the head off. I can do all of these inspections in less than a half hour.
Wouldn't you want to feel the impeller fan for any play? Turbo on these is what? 6k?Brettj3876 Thanks this. -
Valve cover is 8 bolts. You don't care to see what the rollers look like?

About 500k miles, head is a throwaway and between now and a million miles the engine can fail with no warning. At that year I don't recall if they used the updated rings for the carbon packing issue. I have a 2014 isx 2350 and just bought a 2016 with the paccar, simply because I am sick of Cummins' poor design choices impacting mystery business, I have the tools and knowledge, just not the desire to keep wondering when it'll let go.
But! You might be a lucky guy and have a good engine on your hands, or it may have been run at low rpm's and the liners are fretted out and ready to go bye bye. -
What I want and what I get are not the same. You might maybe find a dealer allowing you to take a wrench to their truck but not any I know. I know if I personally was selling a truck you won't be working on it without paying for it.
Some people are dangerous with a wrench. I can see it now, some guy on TTR said to take exhaust off and check for play in turbo and he is in there prying on it with a screwdriver.
I'm sure they will allow you to pay their mechanic to do it though. -
