I had that issue. Checked all engine brake pistons and one of them was loose .50k miles later works perfect.

You have 3 choices. Bring it back completely to stock. Get factory tune and put emulator. Get a good tune with egr system properly working.
Never disable EGR Valve on Volvo engines and Detroit DD series.
Bad tune. Common issue when you disable EGR Valve. Because of high fuel injection pressure and without egr its gets too hot in combustion chamber....
Its 5 years from the date of service or 750 k wherever comes first.
Did you check your air intake system for air leak, charge air cooler, hoses, clamps and change boost pressure sensor.

It does. Injected fuel creates more power energy inside combustion chamber while using engine brake. Do more research on D13 engine.
