Go to YouTube and search Volvo d13a sensor location, there is a video in Spanish but written in English going over all the sensors. As far as repair, disconnect the connection, and use a large wrench, probably a 3/4 inch or bigger or a ratchet with a deep well and back it off and replace it. It will probably need to be torqued to a certain spec, but really a simple fix.

It's at the 6:44 mark of the video, just know that you are going to lose coolant so once you have the old one loose, I'd be ready to swap them quickly. And have a bucket to catch the coolant. It's not a good picture, but you can see that rusted looking pipe to the left of the sensor is the exhaust manifold which helps you get an idea.
