I just got offered a fairly good price on a 2012 Cascadia with the ISX (fresh in-frame with paperwork) and the automatic transmission. I know there are fuel pump issues, in addition to EPA problems (been fixed), what else should I expect?

I have had a 2012 Cascadia for 5 years and actually the last 3 I have had fewer problems with it than the first 2 years the biggest thing about the Def don't idle and run around 70 mph or at least 68 where you can and get dpf filter cleaned once a year ... I have a 10 speed

Make sure inframe includes new or rebuilt fuel pump and turbo and actuator... there's a video on YouTube somewhere a guy spent like 25000 rebuilding his and didn't do the fuel pump. A few thousand on rebuild it quit and his engine is toast.

We have a Cascadia company truck in Indiana right now down for, as I was told, "metal shavings in the fuel pump". Honestly didnt really make a ton of sense to me, as Im not hugely mechanically inclined. But it is gonna be down awhile and cost a lot to fix. They got the driver a rental car back to South Dakota to get another truck and bobtail back to get the trailer. From what I understand, the problem is common with Cascadias around 500k. This isnt our first one.

I’d run from that truck. As mentioned , fuel pump plungers shatter and take out engines ( had 2 different fuel pump plungers shatter , the original ceramic ones and the upgraded steel ones). They had issues with liner drop , although that might be addressed if it had an inframe recently. The only way the EPA problems have been “fixed” with that truck is if it has been deleted, only way to fix that nitemare and I’ll leave it at that. The turbo on that truck cost $7000 , just for the turbo not including labor and the heads had problems with injector tubes crackingand letting coolant into the cylinders . These are all problems I’ve had experience with first hand on a 2012 Cummins ISX , and know more than a few people who have experienced the same issues. I’ve been at dealers and seen trucks that look almost new with 200k on them getting inframes . We weren’t so lucky , when our engine let go the first time at 300k the liner blew apart and took out the block and it needed an out of frame and a new block , that out of frame cost $30k . Also those computer operated auto transmissions have issues and are very expensive to fix. Still interested? That’s a disposable fleet truck that you be sorry if you buy it and make the seller very happy that you did and take it off his hands.

If Shade tree joe did the inframe then I would run away.. inframe with paperwork from Cummins would be worth looking at for right price.. cummins would have done the updates if they are the ones who Inframed it.. I’m not a fan of crapc

This. There are lots of trucks for sale with fresh inframe. Cheap aftermarket parts, not experienced labour and the new owner gets to rebuild it again.

We have 12 and 13 isx. They have serious problems. So much so that the owner is looking at buying Pacar motors this time around.
He is Lifetime Cummins fan.
Lost a motor in one at 350k for bad fuel pump. Cummins put a new crate engine in. Needed a camshaft and 2 injectors 120k miles later.
3 of them are autos. All three have been on the hook in the last 6 months for trans problems.
