You need to sit down and write a letter to the district office of FMSCA in your area. You need to have details spelled out in your letter, such as dates, truck numbers and names of those in violation. Then you need to sign it. If you do not sign it, you have wasted your time. Now, your name will never be revealed unless it progresses to court, which I doubt very seriously it will. So, do not worry about the company ever finding out who "turned them in".

Most Law enforcement agencies are on Facebook. Private message them your concerns and maybe even send some pics or video. Ask to remain anonymous.
I, honestly, refuse to drive faulty equipment. I dont care about the job, there will always be another. If the company threatens retaliation , point to the smart phone camera that is pointed at them and tell them to smile for it. Know you state laws regarding recording and such. Utah is a one party system, meaning only one person has to know -
