Darn thing was blowing smoke rings from the misfires. It was lack of power and what I said, ran badly, the surging sounds more like asphaltene, to me, but if you can drain the fuel water separator or the fuel filters into a clear container, the fuel will appear cloudy black if it's a biological, should appear normal if it's asphaltene.

    Sdatruckinginc and Lepton1 Thank this.

gallon of gas in each tank works good for algae

I saw it 40 years ago. I had the problem with my assigned truck over 10 years ago and never since. I'd say it's less likely now, but still possible in the event someone didn't drain the water off a storage tank that sat unused for more than a few weeks and it somehow got used.

Nothing like mine just loses power random like a derate keep driving power may come back 5 miles or a hundred no codes pulls great when right no excess oil usage smoke or missing a little shudder when power drops is all I’m not a drinker and have no hair to pull out just baffling

What is really bad about algae or whatever you want to call biological crap growing in fuel it is growing everywhere. Asphaltenes can be filtered but algae spores not. It will grow inside the fuel tanks and inside fuel pipes, fuel pump or injectors. In any place it could survive.
You can buy test kit to determine what the 5it you have in fuel system. Just one for example but you can google more Low Cost Diesel Fuel Test Kits for Water, Algae & Contamination
If test come positive for bio then pour double shock dose of biocide in the tanks and fill it full. Run engine for about hour to let the biocide go thru the fuel system. Shut truck of for the day. Run it again for 1 hour the next day. Have spare fuel filters as 5hit will start flowing in fuel system and you need to catch it.
I'm lucky to write this after aircraft where I was the tail operator almost went down. Aircraft was fueled in Cam Ranh, Vietnam and was sitting for about 3 weeks. After we went on patrol mission we lost 2 out of 4 engines. Fuel filter were plugged with algae.
Aircraft was placed out of service to open wings, remove all fuel tanks, all fuel lines and fuel pumps.
    No kidding.
