Sorry, went out of that business years ago. A good printing co should have several templates to choose from tho. Basically what you want is your company name/logo/information, a line for shipper's name and info, a line for consignee name and info, shipping date and delivery date. You'll need a bigger space for writing in load description and approx weight, delivery instructions (time of delivery, etc). Finally a line for shipper's sig and consignee sig. Of course each BOL needs a number for filing. Mine were around standard letter size and carbonless copy.

One of the smartest things I did was to hire both a lawyer and an accountant. Saved my sanity if nothing else.:biggrin_2559:

Now as for permits and stuff, lots of the O/O's here recommend OOIDA (?). I'm sure someone can tell you more about that... or check in the O/O forum.

The logbook is for how far you run also. Anything over 100miles from where you're stationed, and you need to run a log book.

If you will be doing oversize, contact the state and buy an annual permit.
    They are kinda pricey now.
But the permit will allow you up to 12' wide and a certain weight depending on the number of axles.
It will save you time just waiting to get a permit every load.
Also keep in mind like previous poster stated. No favors for friends.
Have your rates set for the lds. you are doing.
There should be a mim. fee for the job.

Each job requires a bill and invoice.

Attach a load number to each one as it makes it easier when billing them.

Also like what has been posted before. Insurance/ surety bonds/ fuel tax reports
    Oh and license plates...

I use to pull a 37 ft Eager Beaver low bed,I hated that trailer. It has the ramps that you have to flip up by hand, and equipment never came off easy. It's been sitting for about 3 years now,and they should make a flower planter out of it.

Can the bill and invoice be used as the same or do I need to different forms?
