trailing too closely in traffic and failed to slow down, hitting another vehicle before hitting the charter bus.
It is one thing to manage or not manage your "space cushion" as I used to teach as a Smith System instructor, difficult as people cut in front of you into your safe area.
It is completely another story simply to not drive your vehicle(or be distracted) and not slow down as traffic demands.

This is a preliminary report from the NTSB which places blame directly on the semi truck driver.
This is not alleged, or there is a possibility of, or evidence points to some mechanical failure.
Unless some other evidence becomes known, which is unlikely, this will be the final finding.
All the interviews have been conducted, ECM's from the various vehicles (if they didn't get incinerated) have been examined, weather was ruled out immediately.
The article details the names of all of the individuals who perished in the crash.
