I am considering a 2002 379 with a Cat and an 18 speed autoshift, but if I buy it, I would want to change it back to a manual. Just what would be involved, and who could do it in the Buffalo-Cleveland-Pittsburgh area?
What would be the expected cost?

may I suggest you try the auto shift a while before you decide to change it.
the current truck I have has an auto shift in it and I remember when I bought it that i told the salesman that i likes the truck except for the auto shift and he said that he would replace it if I didn't like it but to give it a try for a month before deciding.
I am so glad i agreed to this condition as after driving it for just a few days I found that I loved the thing and now after 4 years i would NEVER go back to shifting.
I could go into the advantages of the autoshift but really just try it out for yourself and it will sell itself -
what do you want to do? sell auto shift as unit with all electronics, reprogram ECM ore yoy wanna change tranny cover, add range and splitter valves.... CONVERT AUTOSHIFT TO MANUAL?

The box is the same. If you have the autoshift (has standard clutch) and not the ultrashift (no Clutch pedal) then it's pretty simple. Change the shifter mechanism, thats remove the X-Y shifter and put a lever in. Remove the brain box and install the splitter valve. Then its reprogram the ECM and the VCU<<< it will throw codes if you don't because it's looking for the trans ECM. Granted thats a very simplified version but thats the basic program.
If you have the Ultrashift it gets complicated. You need clutch pedal and linkage a new clutch and you have to remove the Inertia brake from the trans. Then you do the rest of the other stuff as well. And you need a bellhousing so it will have the holes in it for the clutch linkage to go through.
We looked at doing one on a 377SFA that has an Allison world auto 6speed. It wasn't looking to be cheap by the time we would get done.
One last note, some will say you can't run an auto box as a manual. I do not know one way or the other. I do know that the Ultra box is different internally from the auto box. IF I was doing this I would start with a box that was a full manual from the start instead of doing a work around and converting something. -

Doing this in your garage at home versus a shop doing this, is the question. I do not want this to sound like its a pull a few bolts and change a couple of wires around and you're done. We both know it's not that simple.
And a guy doing this at home or a tech in a shop working on this stuff all the time is very important. Many of the guys on this forum are very mechanically inclined. But they fact is they don't deal with a wiring or in this case a rewiring mess on a daily basis.
Plus they will have to find someone to reprogram the ECM and the VCU. Due to liability issues and downtime issues (who pays if the truck won't move) that little reprogram might be harder to accomplish than you think. -
I've forgoten to say do not forget to add and program range behavior valve. Just imagine! You drive at 50-70 MPH and you accidently toch lever and tranny shifts down to low range (1-5)....

I just inquired on the same procedure for my 2005 Volvo.
To go from autoshift to manual a dealer quoted $9000, a specialized shop quoted $6500-$9000
This gives you the range.
On weighing pros and cons I decided to trade mine in as is.
Not sure about you, but if it were me, that would have to be a pretty special 2002 truck to spend this kind of money on. I'm sure you can find the same year/condition truck with a manual and save yourself some headache. -
I am surprised at that figure. I thought it was somewhere around 2-3K, considering the truck already has a clutch pedal.
If your figure is accurate, then it's a deal killer for me. -
I don't think I would mind one with a clutch pedal, not having a clutch pedal isn't good,at least not in my experience.
I'd pass on it if I had to have a manual transmission.
