    It looks as if perhaps the loader wasn't tied down yet, and maybe the truck brakes were not set and it began to roll. Although I don't know what stopped the truck unless it was the loader tires engaging the drive tires. I am looking at this on my phone so maybe I am missing something.

    My first thought was "Oh, he didnt cover the stack" Then I thought, he hit the throttle instead of the brake like what happens to all those people that use their right foot for the brake (auto trans). Or it's one great photoshop.

    Yeah, the shading/fading on the loader doesn't seem to match the rest of the picture.

    I think that visor mght be a little too low to satisfy DOT Regs.

    No windshield cover on the loader, he's gonna get stone chips with open bolsters...

    If I can find them I have more pics of the wreck. It was a fatality, the driver was an experienced driver I believe they said it only had two 3/8 chains on it and he had to brake real hard. I'll see if I can find the article.

    He forgot to unhook the neck before loading.

    Not sure how they pulled that off,
    or I should say
    Got that up, there!!

    Is that a binder on the ground and the truck tires are chalked? I think...
    A Crane was involved... whether it was a Problem or Staged?

    Yes that is a ratchet binder on the left side half cut off by the frame of the pic, and I agree the drives are chocked.
    Problem or Staged...not sure which, but something is fishy!
