broker agreement example or template ?

Im going to start pulling some of my own loads off load boards dealing with various brokers. Im wanting to write some simple agreement form of my own to state detention time, lumper fees, ect to cover myself as best as possible. Trying to find some examples i can use to make one up for myself. If anyone can help me,shoot me a link or message that would be great. The broker im working with now is shakey at best, always pays me but i have no real contract to work from, and now has only one way freight, im returning empty good pay but still empty. Anyway thanks in advance for any and all help, and for all the good info on this site, tried a search but had no luck (could be my methods though) THANKS AGAIN

 

Called ooida they had one ready to go, fill in the blanks or change it as you want. I had to call them could not find on their site, membership fee is well spent once again.,
