Can employer require your Medical Long Form?

    I have had some companies who pay for your physical have your long form sealed in an envelope at the doctors to be returned to the carrier. Make sure you gave them permission and you have all rights to the original. Current regulations require you carry the certificate and all supporting documents. I would think the long form and any waivers supporting documents.

    What you are talking about MNDriver is on the state level. The federal regs will start Jan 30, 2014. You have to satisfy both until then.

    Not a stupid question at all. The state requires an updated medical card when renewing your license and with the new law will be keeping those in a database so depending on your state, you may have to take it into your local dmv when you renew every two years.

    On the federal level, you are required to always have your dot physical card on you. I keep mine in my wallet with my license. Part of the new law is that a national database of approved physicians is being set up and you will be required to go to a physician that is registered in that database to get your dot physical done. I believe this takes effect May of 2014. They are weeding out those physicians who will pass anyone just for the money. I hope this answers your questions.

    Once your certificate is filed with the state, you are no longer required to have your certificate on you.

    I agree. Plus not all states have implemented this yet so those who assume that it's all good are incorrect.
