Well let me give you the whole story then.... I bought my first lipe about three years ago after reading on here how good they were and thought it would be the right clutch for my application seeing how my trucks haul permit loads up to 140,000 lbs. About eight months after putting in my first one the throw out bearing started to vibrate so bad it broke the grease line off. So I pulled that clutch and found the bushing inside of the throw out bearing that rides on the input shaft was just about gone. I called setco and talked to a rep that informed me that they had problems with that and they updated to double bushings one at each end instead of just one on the clutch brake side and to make sure I had got the updated clutch from my distributer which I did this time. So I went ahead and installed another lipe in that truck along with a few more in the next year. Now I am going through and replacing them again for the same issue although they did last a bit longer with the double bushings. Just a FYI I service my trucks every 10,000 miles and the bearings get greased with high temp grease every time. Also I might add that I have no other throw out bearing issues on any other brand clutches that I run. Also another local trucking company was using them until he had a throw out bearing blow apart on him.
I hope I answered all of your questions if not let me know and I'll try to give more details.

Currently running Lipes in two trucks with more than stock horsepower and no issues at all. One has been in service over two years now, the other less than a year.

Have heard early version Lipes had bushing problems on throw out bearing. One guy I know had this issue after about two years of use. He called Lipe dealer and they admitted the problem. Cant remember particulars but he got a new clutch and was overall satisfied with what they did for him.

And it depends also, were those Lipe 1 or Lipe 2 clutches power freak? Different ratings for each.

They were lipe ones. I did call setco and got them to send me a new throw out Brg for one of there failed units and I put it in, it didn't last either. Although I must say that I have two units with them still in and They have over a couple hundred thousand on those clutches and no problems.
