DAF and VOLVO trucks were used as well.
i even saw an argosy once with this setup.
they arent factory, a local company ( i think TruckART ) manufacture and install them.
and your right, no fat drivers would fit.
there wasnt a ladder in any that i saw, had to stand on the waeco fridge to get up..
guess all the pull ups helped keep them slim.

i see larger problem to drive it than to sleep in this
such short tractor could be very dangerous as much i remember i read on lkw infos that they have 2.9m wheelbase or even shorter
related to cab size
my father drives it about 17-19years ago and he says that is impossible to sleep inside while other driver drives it ( cab is too high and it swings too much)
this insanity was also used in EU for many years ( from 70s to early 90s)
shortest cab was 52inch long
distances between truck and trailer was smallest possible all to maximize load volume
some trucks are going to extremes so much .......
so cutting cab was quite normal
removing cab suspension( to minimize space between cab and bodywork ) was normal
some trucks have engine not below cab but between truck axles , installing front air suspension retrofitting trucks with steerable tag axle
hydraulic drawbar which extends while you are cornering or steering -to minimize space between truck and trailer so some trucks have only 1foot between truck and trailer ( this is for rigid/straight trucks +drawbar trailer-pup)
also lot of conversion on fuel tanks lowering truck suspension cutting mudguards changing air intake and exhaust to create more room for bodywork and also gear knob .....
those trucks were particularly dangerous because of their short cab in collision you could be trapped inside in this time 41 pallet was quite normal
40was minimum (with central axle trailer) -
I think it's only practical on these kind of trucks.

do you know how long is this BDF body on truck or trailer
as much i know they used 8050 in 80´s but since now is 18.75m this could be even longer -

like 8050 ( with short cab) i have seen 8200 8500 and 30ft bdf
( once 4x2 with 20ft and 3axle drawbar trailer 8200 or 8500 something like this ) -

what if one drivers sleeping and the other ones driving and they hit a low bridge
that would be a hell of a way to wake up -
in case it is factory bed like in scania topline series 4 have there are chanses you will be live
usually in trucks with those short cabs and high this is plastic part but in those trucks in 99% cases only one driver , there is not enough space for both drivers
and also those extra short cabs are really dangerous because in crash you could be trapped inside
