Not just dac. 391 gives a whole list of things like employment history, positive drug tests, wrecks, and more.
Dac is just easier for large companies as they contain all the required info in one place.

i was in the habit of getting my Yearly FREE DAC report, just for my own self, to make sure everything was A.O.K.
surprised me too the first check, then later the second, then the third....

If you hit something that has no life in trucking with a 18 wheeler, you have failed in some way to avoid same. Its a game of inches in some situations. Getting out and look, staying alert and aware of the problem at hand, manuvering etc. Anything can happen.
Companies will run drivers off and fire em with two or three preventables which is what hitting a post, pole, or whatever that cannot get out of your way, and usually BUILT to keep vehicles OUT of some section of a lot or dock.
Im not your opponet. But all of us (To paraphrase) have hit something at least once. Those that cannot stop hitting stuff well... they cannot continue with trucking.buddyd157 Thanks this. -
This compliance is critical. The FMCSA when they do a safety audit these files are some of the first files they seek. I have said this before and @brian991219 has as well. When there is an audit these auditors already have a "hit" list of problem drivers taken right from the data from the state inspections. They will hit these DQFs as well as their HOS records. If these files are sloppy or incomplete it can spell serious trouble for the carrier. This information can also be required by states if they audit, AND can be subpoenaed by an Attorney as a part of discovery in a lawsuit!brian991219 and Badmon Thank this. -

I learned some hard lessons running for Werner, and I'd like to think I can improve based on those lessons. One thing that they marked as "preventable" on my DAC was legitimately not even my fault. The Driver's Side ferring got torn down because some idiot field service tech replaced my lines then added a third tethering spring, and attached all the springs on one individual bracket. I was a driving a 2019 International LT. The brackets themselves were plastic. All it took was some tension at a tight backing at a later stop to cause the brackets to buckle and fail.
Now I dunno about you, but when I go through my licenses and certifications, I don't see "certified in air-line replacement/repair" written on any of them. As far as I could have known, the repair was a good repair.
As for a downed cable (that I honestly could not see, and had no signage or lighting to make it apparent that it was there, could not distinguish it from the horizon line at that time of night) parking lot screwups are a thing. I own where I #### up.
Could I have set my breaks, put on my 4-ways, left the truck on the road and checked out the area on foot before proceeding? It's a possibility, and if anything happened to that truck while I was doing that, I'd still be getting the blame. Could I have called? Possible, but the phone numbers fro Dollar General stores are 90% of the time bogus, and when I say 90% I actually mean they change their numbers more frequently than their underwear.
So does Dave have a foundation to stand on? Maybe by virtue of his experience, however he asked a question, and when I said "I posted it elsewhere" to come back and accuse me of being lazy and incompetent are both grave insults where I come from.
So to answer his question; Werner contends I was fired because of "unsatisfactory safety record", which was only called into question the day after I requested transfer away from the Dollar General account, to something less risky. I'm not a bad or careless driver, I GOAL like anyone else, and in this case, I had some bad luck with a couple of non-reportable parking lot #### ups dealing with a crappy account that gives you no intel and puts you in situations you likely would NEVER face running facility to facility.
Hell I hauled trailers into 2nd Street in the heart of Philadelphia with no issues, backed into spots at truck stops and crowded lots without ever making contact with anyone time and time again in the short time I was with Werner. I CAN do the job. My issue is that I ran for a dirtbag Mega, and because I told them I didn't want to haul their crappy freight anymore, they decided to burn my ability to find work elsewhere, keep my from hauling for competition. It's what they do, they have a reputation on this forum for doing such crap.
Hell I could drive through a ####ing ghost on the interstate and Werner would rule it as a non-DOT reportable/ Preventable Incident on the DAC.Attached Files:
