Question: A driver does a job, has standby and or uses pumps, hoses etc. and either fails to write it down ( at all or wrong ) or misses a known item to the carrier... In your opinion does this item and it's charges become the property of the carrier to bill and keep all collected monies pertaining to this item as"fair game" or should the carrier correct / add the deficiency and pay you your due ?
(eg: forgot section of hose or went 15 mins over free time etc)

My opinion is that you should get what is rightfully yours.In the real world its just as likely you won`t.

I would say the carrier can only bill from what YOU, THE DRIVER, informs them about. You are the one at the job site with times etc. If you turn in your times and they are wrong, it's not their problem.

Fair game or theft? -
With my terminal though demurrage does and accessorial charges do not require a separate request . If we forget to note using a compressor or other accessory and the clerk knows that is normal procedure she will call us to confirm then add it . -
As it should be IMO. A team effort...... and yes on the payroll and yes an extra slip is required with signature PLUS if the customer refuses to sign you must get a name and why....like co. policy, boss is mad or even my fav " I don't want to "
