    I wouldn't mention it unless asked. It's no big deal; if you pass the physical, you're hired.

    I've been living with Type II for 14 years now. You're doing great. As was previously mentioned, if they don't find high levels of sugar in your urine you're good to go for a one-year card. Your medical condition is between you and the health professionals you deal with. You needn't disclose it to employers unless they have an in-house physician for preemployment DOT physicals... you are required to disclose preexisting medical conditions by law on a DOT physical.
