I’ve never been checked for scars. I’ve lied on every physical i’ve ever had. Yes i’ve suffered from broken bones, yes i’ve had numerous surgeries, yes i took a stupid sleep study & wow they said i have sleep apnea.
i answer no to broken bones, no to surgeries, and no to sleep apnea/snoring. Get a 2yr card every time.

Seriously, I've had about 1/3 of the docs or nurses looking for a scar on the chest or abdomen and maybe 10% physically check for a hernia. <cough, cough>Val_Caldera, Sirscrapntruckalot and JoeyJunk Thank this. -
You used to be able to buy blank physical cards and annual inspection stickers at the 76 and fill them out yourself.

I've had both knees operated on, way back somewhere around 1980.
3 hernia operations around 2004-2006.
All are listed at every physical.
Although I don't remember the names of doctors or hospitals, exact dates and such.
I just list them, because they happened.
And in these 'modern' days of interconnected everything... it is more of a risk not to disclose the truth.
Just as it is a risk to say anything bad about your company on any social media site, or admit to any wrong doing on any social media site, or show anything about yourself in social media that your company (or government entity for that matter) might not like.
And it is a wonder that so many people still have not grasped that reality. -

In this case it sounded like the OP disclosed a medication regimen. The medical professional involved concluded those were medications that were typical for conditions he didn't put on the form. They questioned him further, he provided no additional information. They refused to certify him and are required to send the form to the Feds with that information on it.
At this point that information is in the national database, which is a mess.
If he does another physical there is a question that asks "Have you ever been denied a DOT certification in the past?
The answer for him is yes.
Go from there with the new process, disclosing everything. -
When I get a physical and the forms also include the signed self-certification form for my state, why are they not also sent to my local DMV?
I should not need to take pictures with my phone and e-mail them to myself, fill out the same form online and then figure out how to transfer that to my computer as an image, and then upload all that to the DMV.
It is already in the system.
