usually, they do a quick check before orientation, then the full blown check when you get there. they may or may not re-call previous people.
at least, that's the way it used to be.

Not a good driving record for the length of time you've been behind the wheel. At the rate you're going you'll be out of the driving feild and changing occupations. Which could be a blessing in disguise. It's hard to face personal failure but at the same time a learning experience. Nothing to be ashamed of everyone has a certain level of competence. Good luck on your next adventure.

Bottom line! Tell the fricking truth on that #### application! If you have had 40 at fault accidents over the last 18 fricking months put that on the #### application IF asked. Then go read the #### FMCSA rules about what a carrier has to do when employing you. They have to build a fricking driver qualification file. They have to account for all your time with a CDL going back at least 10 years (if you have that much history). Once you tell the truth and get a call to head off to orientation you can stop the worry. Heading this crap off starts at the application. Answer those questions fully and truthfully. BUT NEVER volunteer information not asked for.
