Yes, in fact the last page of the long form is the medical certification by the examiner of your fitness to drive. I had a DOT officer in South Carolina to ask me for my long form.

In cali, you are considered driving out of class without it. That is a misdemeanor.

The clinic where I had my physical made copies of the 4 pages, made the copies smaller, and put them all on one page. Folds up nicely in the wallet.

Really? I've never been asked for it. I keep it with me just in case though.

Per the law you are no longer required to carry the medical card after you take it to the DMV for them to add to the system. I was pulled for a Level one out on I5 and also on I15 in cali and i was not asked for the card either time. now that being said I still keep a copy in my wallet cause i dont trust DOT to know the law. As for if its to big of a paper just go to a copy machine and make a copy and shrink it to wallet size.

Will someone be kind enough to quote the section of regulation that requires you to carry a copy of your long form, bet you can't. In 50 years I have never heard of anyone being asked for a long form, just the card.

You are absolutely 100% right, never the less I have been asked for it three times within the last year. My policy on that is show them, just because you are right doesn't mean you won't get a ticket or even worse OOS. I had a Nevada DOT give me a level one and when he checked my logs asked me why I did not fill out the from and to at the bottom. I respectfully replied that we had not been required to fill that out for at least ten years. You would also be amazed how many LEOs don't know the DVIR is no longer required.
