    I do you have to self certify every year in pennsylvania, or do you do it one time and it is on record with them?

    One and done until you decide you need to self-certify a different intended use for your cdl. Example, my wife self-certified excepted intrastate about 15 years ago and hasn't taken a physical since but still maintains her cdl and all she would have to do to go back to non-excepted interstate is show PennDOT a valid medical certificate and fill out a new self-certification form.
