The roads and rails of the US are not that way, they are built progressively as traffic demands grow, retrofitting road to rail as they go along.

Interstates are built to a standard but many non standard roadways exist. Rebuilds need to meet the standards, good example is the PA turnpike where any rebuild or new build [tunnel bypasses] meet standards while the older sections have almost no center shoulder room, etc.

Got hung up on the train tracks in compton ca, once. My landing gear was hooked and I could not go forwards or backwards. The fellow who answered the 911 pushed the airbag control lever to over inflate them and I was able to back off. My trailer had 22" wheels.
