DOT Long Form Q.?

    Hello,Ive been OTR sence 2000,all this time and with the companys ive drove for,its been my understanding that I was suppose to keep "my original long form from my phical " in the truck with me. The guy im driving for now tell's me he is suppose to keep it in his office. Ofcorse I know to keep the wallet card on me,but does anyone know about "the original not a copy of the Long form". Thanks for any info....

    Far as I know its one or the other, but most guys carry ONLY their wallet card.

    I have always carried a copy of my long form physical along with the card in the wallet. I have had the DOT ask for both.

    I'm surprised they would ask for the long form as that is private medical history between you and your DR. I would tell them that it was none of their business!


    And when ever I've been asked for my proof it has always been "let me see your medical CARD".

    Not really. If they need to confirm medications, it is there on the form.

    Still aint none of their business! They are not trained DRs.

    The prescription bottle with label is all they need and will get from me.

    There are drivers that will not tell the DR exactly what medications they are on in order to get a longer physical. If the medication in possession has an issued date prior to the physical form, and not shown on the physical, it can be found to be invalid.

    There are also states that issue only a long form and no wallet card. In those states, the DOT asks for the long form.

    You keep a copy of the long form and you should have a medical card.
    If you don't then you can be put out of service and or written up.

    The copy of the long form gives any issue's you may have.


    Again I am going off memory here but I am pretty sure I am right
    The original must be kept at the office for DOT I do believe
