    The mandate shows.

    Compliance

January 30, 2015 States must begin enforcing the regulation that paper copies of the Medical Examiner's Certificate are only valid for the first 15 days after their issuance

    <<>>

DR's giving the Medical Cert are required to keep a copy for 3 years.

Your Medical Card and Long Form are the property of whom ever the physician issued it to! It is
against state and federal law to with hold a persons medical record! Not only that you are suppose to
have the long form with you because they request to see your long form! Also, If you have been
said to have sleep apnea and are on a C-PAP machine, you are too have a copy of your Longform on
you along with you sleep apnea doctors name and phone and a copy of your 90 day download!

No you do not... paper forms are invalid after 15 days. The Self Certify puts this in your MVR so the LEO can access it. This was the whole idea of Self-Certify. Medical forms are now on the MVR and in the system.

Future - Medical Officer will turn the information directly to the state and the driver does not have to do anything.

Regardless who paid for it you signed for it at the doctor's office in your name go back and get a copy it's your health record and by law it attaches to your drivers license.

Nope Because it attaches to your drivers license. Give it back to them and go to the doctor's office and get a copy it's your medical record you signed for it regardless of who paid for it. It says your name on it doctor's office knows you as the patient.
