Is it customary for an employer to request the Long Form Physical? I was under the impression that the detailed info that is listed on this form is confidential.

Yes, it's common, but seems not every carrier requires it. Yes, there is medical information on the form that you may consider confidential, but there is no law against them asking you for it or for you to provide it to them. You don't have to provide it, and they don't have to employ you if you don't. HIPAA does not apply in this circumstance because it's not a doctor disclosing the information.

Who gets a short form these days?
Isn't the employer always required to have updated med and license? -

More than one carrier has been slapped with financial loss, and cease and desist from courts about being too nosey in driver's medical history. It's all on the internet.

If the employer pays for the physical, they basically have the right to a copy. What's on your physical that's a big secret ? Just curious
