So if I buy a truck they still has a factory warranty, let's say I break down many states away from the state I bought the truck. I get it repaired, do I pay cash and the warranty reimburses me with a check in the mail or do they confirm they warranty covers the cost upfront before the repair and there is no charge?

The aftermarket warranties have towing in them too.
It's part of the basic coverage. Because some items require IMMEDIATE shut down, or you'll lose the engine.
Think mine pays $1,000 in towing.
