Epa07 Detroit diesel 60 14L white smoke

Hello, I am having issues with my 2008 cascadia 3.58 rears 10 speed. I am getting a fault code (ecu 128, pid 412, fault 02). The yellow engine lamp is on "not the one with the engine in the middle but the solid yellow lamp". When I down shift going down a grade and start to go back up and up shift the truck starts to hesitate and blows out huge puffs of white smoke briefly when accelerating. What could be the cause of this? it's getting worse. Does anyone know what the fault code reads? I have lost power also I can barely pull 39k up hill.
