Michigan's actually works fairly well. I know, I'm surprised as well.
It's simply a web form. Scan the long form to pdf, attach said pdf to form and click submit. You can even verify it "took" at the same site. Takes less than 5 minutes every time I've done it.

We're lucky around here. It's about a 3 mile drive between clinic and DLD.

I don't know if there's an online link.
