Failure to report prescription meds on DOT Medical form for 2 year wallet card

    The driver said he discussed it with his doctor but didn't write them down. I'm not sure why the doctor would put him or herself in jeopardy by signing this page. I've never taken a medication so just blow my this page without paying any attention. However, it does say to list and discuss but not that if any are listed you can't drive. I think the implication is the doctor will use his judgement and not approve the form. If he didn't list it but told the doctor and the doctor still signed the form I'd say the doctor has a giant problem also.

    Look at the form at the link provided by 123456 . If the driver didn't write the prescriptions down in the area provided the examination can be ruled void and civil penalties levied .

    Kinda where I was going with this but didn't state it clearly. Everyone is liable in this case.

    The Form speaks for it's self. If it ask then you must write it down.

    ok...just took a DOT pre employment drug screen a few days ago.....this is what it says


    "after the medical review officer receives the test results for the speciman identified by this form he/she may contact you to ask about prescriptions and over the counter medications you may have taken. Therefore you MAY want to make a list of those medicationsfor YOUR OWN records.THIS LIST IS NOT NECESSARY. If you CHOOSE to make a list,do so either on a seperate piece of paper or on the back of your copy."


    the part where it says "this list is not necessary" in capital letters, is in capitals on the form itself....

    this is info printed on the drug test form under step 5....under your signature...if i had a scanner i would scan and post it.....

    Pre employment drug screen has nothing to do with the long form medical requirements.

    More than you think.

    A guy I knew, hurt his back and took some pain meds from a friend. The next day, he had a pre employment drug test. They requested his prescriptions and the meds were not on it.

    He failed the drug test.
