SO the instructions say DO NOT WAIT to mail in your haz-44 form to the dmv but you must mail the dmv a certified copy of your birth certificate along with your forms. The fingerprinting place wants to see your birth certificate so how am i suppose to mail it in before I get the finger printing done? If I mail in my birth certificate do I get that back? Or do i have to spend 100 bucks and order a new one?

I would think if you take your birth certificate to a notary, they can copy it and certify that it is an actual copy of the original certificate. That would be a certified copy of your birth certificate and would be acceptable. Most bank branches have one of their staff that's a notary, so it shouldn't be hard to find one.

ok i just called them up it can be a photocopy the paper work says it must be certified but when i called it just has to be any copy. They need to change that form because it says certified copy.

Just to be safe and to save you time and the cost of another stamp, get a certified copy. When you send anything out with your personal info, make sure you send it with a signature confirmation that gets sent to you when it is signed at the receiving end (USPS). Fedex and UPS will also send you the actual signature, time and date of delivery. It also gives you a record of the date and time it was received so when the DMV says they didn't get it, you pull up the confirmation and tell them otherwise.

the fee has dropped to $86.50.anyway,the driver MUST complete the TSA hazmat driver application before going to the fingerprint location.
I just learned of all this myself so i, rather ignorantly, walked on into the closest cdl license facility thinking i could do it all through there.NOPE.should never have allowed my hazmat to lapse 15 years ago.it is going to be fun reaquiring it.
the closest fingerprint facility for me is going to be 130 miles.I am doing this on my own so i have plenty of time i reckon.
I posted this for those like me that mistakenly thought the hazmat endorsement would just cost the background check fee.ha. it is going to cost time and money. -
