    I'm going to be a little mean.

    As a former safety critter I would have defenestrated you after the 3rd one.

    On the dropped trailer, if you followed the correct procedures, the trailer shouldn't have been able to tip over. If it was just the weight, it would have done it while getting loaded. If the landing gear collapsed, your rear drive should still have been under the trailer to catch it. My guess is it was your tail pup and you didn't crank the landing gear up after hooking. What was a 1-2 inch gap where you broke the set turned into a 4 inch gap at the dock, sharp drop and timber.

    The cable I'll give you - it's a live and learn situation. Always look up for cables/trees/whatever - particularly when on property that doesn't get trucks every day.

    The brake valve - that's 100% on you. Did you not question why there were no dummy glad hands? Without them or something else blocking off the red glad hand you wouldn't have been able to position the lead to hook the dolly/set. Then as you were pulling out you should have felt unusale reistance, the tail wouldn't have followed out nicely, and you should have seen/heard the tires dragging. What this incident tells me is you aren't paying attention. If this was the first incident - okay, I'm going to yell at you and move on. Third incident? That's a pattern and you're highly likely to do something catastrophic that will cost me my job - so out you go.

    For the dock - you may have just been unlucky, but a standing rib roast to Lime Jello Fudge you didn't hit it square and center, or you hit it way too hard. Either way, with 3 other incidents it's a pattern that is problematic.

    Going forward you need to pay closer attention to what you're doing. Notice when things are off, stop and make adjustments. Best of luck.
