Hey guys / gals, ACL Surgery and DOT Physical Question, also new company question

First, thanks to anyone in advance that has an answer to my question's... Here they are ( I apologize if in wrong threads ) I'm New!

I'm up for DOT physical in a couple weeks and had ACL reconstruction in my right knee about 7 months ago and have had full recovery / no pain / no problems what-so-ever but was just wondering if the Med. Examiner would require some kind of doctor release for this...?( also had my ankle screwed back together back in 98 and they never seemed to care for doctors notice ) I had surgery through VA and they are very very slow in doing anything so just wondering if someone out there has gone through same situation and actually needed something in writing for a knee surgery or is just everything cool as long as you can run in place or whatever they want me to do this time... Honestly, it healed so well, I wouldn't even mention it on the forms but the scars are there and I don't know if they would notice them and know or not or even care. ( did not come up with much at all with google which is good I guess )

And the big 2nd... Have not driven OTR in about 3 years... lets put it this way, 1 year 2 months actual verifiable OTR in the last 5 years.. My options seem very slim.. I need to get back on the road and looking for some advice on where to go.. Quick backround, held CDL A TX END. since 99.. Drove mostly local with some OTR until 08.. drove OTR until 2010... Yard Hostler 2 years... time off.. blah blah.. surgery... blah blah.. worked state job for DOT until a few weeks ago and quit because running across the free-ways during rush hour to pick up rubber wasn't as fun as I imagined it would be. So now I have noticed that everyone wants recent experience.. The best I came up with is so far is Werner with 2 weeks with a trainer and then .26 a mile.. Problem is recent experience, does anyone know before I start sending off 20 apps or wasting time in orientations / calling tons of trucking companys. ( switch that ) . Is there somewhere I should try first? I will not do the Western Express thing ( what a joke ). will not do swift, crst, jb hunt, prime, navajo, etc.. Some may say to look into smaller line jobs or OTR companys but I need a mega-carrier for the next 2 years that will not screw me too much.. Is there any that is not so anal about recent last year or whatever driving?

I live in Las Vegas and have no accidents / dui / felony / misdemeanors etc etc... Thanks again
