DAC builds their database by rebating account members a small fee off their bill for every 'termination report' they receive It is a selection of multiple choice questions that have been refined by DAC's legal department after several lawsuits and federal regulations under the fair credit reporting act.
The worst two choices are:
Quitting under load or abandoned equipment.
and
Personal contact requested.
When I had 165 owner operators with over 100% turnover, you could bet that I filled one out for each guy who moved on...
I did use 'Personal Contact Requested' for one guy. He was a problem at that time, possibly Bi-Polar.
Wasn't too effective, he always worked and around 6 or 7 years later he and a team partner saved someone's life and he ended up being a 'Goodyear Highway Hero'.
OP, Don't worry about DAC thousands of carriers don't use them, I haven't been associated with a DAC account in over 10 years. I do use the PSP system.
On the other hand, I always answer the previous employment checks I receive with the correct dates, Drug and Alcohol answers if less than 3 years old and major +/or multiple at fault accidents and no additional comments.
In the event that a driver who was on board when I started with the company who was a total problem happens to leave; he gets the same treatment though If I receive a form from someone I know at another company [and there ain't too many out there!]; I will pick up the phone. Only did that one time....

I used to work for a company that was notorious for reporting everything to DAC
I had one at fault incident. Also the reason I quit that company was because they cheated me out of some pay. When I returned the truck, in a fit of rage, I took a knife and made some slashes to the interior upholstery along with slicing the wires to the camera.
They never reported anything to my DAC. I wonder if it was an over site, or if they felt guilty for cheating me out of pay. -
